HOW DO I PREPARE FOR MY VISIT?
Prior to your appointment, it is helpful for you to set up an account on my patient visit to fill out the teledermatology consent forms. An informative email will be sent to you covering account set up at the time you make your appointment. If you are unable to access this, we can do these forms together at the time of your visit.
You can upload important documents such as insurance cards, photo identification, and images of your skin conditions through the portal that go directly into your chart. If you are unable to access the portal, you may send photos prior to your appointment to email@example.com. Please do this the day prior to your appointment day. If you have an urgent dermatologic issue after normal office hours, you may email firstname.lastname@example.org.
